Premenstrual Dysphoric Disorder (PMDD)

Woman lying in bed experiencing discomfort or pain.

What is PMDD?

Premenstrual Dysphoric Disorder (PMDD) is a severe, hormone-sensitive mood disorder characterized by significant emotional, cognitive, and physical PMDD symptoms during the luteal phase of the menstrual cycle. This phase typically occurs one to two weeks before menstruation and often improves shortly after the period starts. Common PMDD symptoms include severe irritability, mood swings, depression, anxiety, feeling overwhelmed, difficulty concentrating, fatigue, sleep disturbances, changes in appetite, and various forms of physical discomfort.

Specific PMDD symptoms may involve:

- Severe irritability or anger

- Mood fluctuations

- Feelings of depression or hopelessness

- Anxiety

- Difficulty concentrating

- Fatigue

- Changes in sleep or appetite

- Breast tenderness

- Bloating

- Headaches or migraines

- Joint or muscle discomfort

Diagnosis of PMDD typically involves tracking symptoms across multiple menstrual cycles to confirm their timing and pattern. For those seeking PMDD treatment, various options are available, and accessing psychological services can be an effective part of the management strategy.

PMDD, which stands for Premenstrual Dysphoric Disorder, is mainly diagnosed by observing the pattern and timing of PMDD symptoms throughout the menstrual cycle, rather than just relying on abnormal hormone levels. Nevertheless, hormone and laboratory testing can still be valuable for assessing other medical or hormonal factors that might contribute to or resemble PMDD symptoms. In these situations, accessing appropriate psychological services can also greatly enhance PMDD treatment.

Treatment

The evaluation of Premenstrual Dysphoric Disorder (PMDD) begins with a thorough assessment of your PMDD symptoms, menstrual cycle, medical and psychiatric history, medications, and overall health. When clinically appropriate, laboratory testing may be reviewed or recommended to assess factors contributing to mood symptoms associated with PMDD, such as thyroid function, anemia or iron deficiency, vitamin deficiencies, and reproductive or hormonal issues. In certain cases, timely hormonal testing may offer additional clinical insights, especially when symptoms of perimenopause or another endocrine condition are present.

PMDD treatment is personalized and may involve medication management, including SSRIs taken continuously or specifically during the luteal phase, under the supervision of a medical professional. Medication management may also address co-occurring psychiatric symptoms, alongside lifestyle interventions. Supportive psychotherapy and collaboration or referral to gynecology or other specialists should be considered, particularly when hormonal treatment is necessary.

The ultimate goal is to develop an evidence-based PMDD treatment plan that is tailored to each patient by understanding the complete clinical picture—not simply a single hormone level—while addressing the full spectrum of PMDD symptoms and the necessary psychological services.

At Sol Psychiatric Care, we collaborate closely with each patient to create a personalized treatment plan that specifically addresses PMDD symptoms, focusing on relieving those symptoms, promoting mood stability, and emphasizing relapse prevention. Our mission is to enhance the overall quality of life for individuals coping with Premenstrual Dysphoric Disorder by providing comprehensive psychological services that support effective PMDD treatment.
